Club Registration Form
The OTTA recognizes two types of affiliation:
- Affiliated club
- Affiliated club with full OTTA membership policy (each club member becomes OTTA competitive member by joining the club)
To ensure we can process your requests for insurance efficiently, please be aware of the following:
- Provide Complete & Accurate Information: When submitting your request, you must include the full legal club name, the correct landlord’s name, and all related addresses. Incomplete forms will cause delays.
- Administrative Fee for Corrections: To reduce the need for re-submissions, a $50 administrative fee will be charged for any request that requires a new insurance certificate to be issued because the initial information provided was incorrect or incomplete.
Our insurance broker has urged us to be more diligent in collecting this information upfront. Your cooperation is crucial in helping us streamline this process and avoid unnecessary fees and delays.
